The purpose of the PATH program is to reduce Opioid drug use, and the misuse of drugs, in individuals who struggle with Opioid/Substance use and to provide the best community-based practices for substance use treatment services. Additionally, this initiative will help all persons served to meet their full potential as independent and fully productive individuals.
This program treats people, not symptoms. Underlying causes, unique backgrounds, and personal triggers all share blame for the disease of addiction. Each client is different. We understand that the path of sustained addiction recovery is through unique personalized treatment in the comfort and safety of our community
Our Comprehensive Approach
We believe in engaging, educating, and connecting individuals with local resources to help break and prevent the cycle of addiction. Our goal is to rebuild and strengthen families and bring hope to communities. During our program, we provide or connect you to resources for:
- Short-Term and Extended Support: Tailored support to meet your immediate and long-term recovery needs.
- Addiction Education and Treatment: Comprehensive education and treatment plans to help you understand and overcome addiction.
- Life Skills Development: Training to help you develop essential life skills for a successful and fulfilling life.
- Job and Career Development: Guidance and support to help you build a rewarding career.
- Job Placement: Assistance in finding employment opportunities that align with your skills and recovery goals.
- Community Resources: Connection to local resources and support networks to aid your recovery journey.
- Case Management: Personalized case management to ensure you receive the care and support you need throughout your recovery.
Admission Criteria
To ensure that our program can provide the most effective care, we have specific admission criteria:
- Adults must be 18 years or older.
- Must have a primary diagnosis of opioid abuse or opioid dependence disorder.
- Must have actively used opioids within the past 180 days.
- Must commit to actively participating in treatment goals as prescribed by the treatment plan.
- Must be willing to adhere to a dual treatment approach with medical and/or psychiatric care.
- Must submit to random drug screens.
- If using medication-assisted treatment (MAT), you must be able to provide the most recent prescription and all prescription refills.
